Yes. If you have an MBBS from an NMC-recognised college in India, nothing stands between you and USMLE Step 1 or Step 2 CK. In fact you did not have to wait for the degree at all, which is the part most people get wrong.

What ECFMG actually requires
The eligibility wording matters, because it is more permissive than most students assume. To sit Step 1 or Step 2 CK you must be officially enrolled in, or a graduate of, a medical school listed in the World Directory of Medical Schools as meeting ECFMG eligibility requirements. That listing has to carry an ECFMG eligibility note.

Note what is absent from the Step 1 and Step 2 CK rules: no minimum year of study, no internship requirement, no NEET-PG equivalent. Enrolment is the bar.
The 2024 rule that changed things for Indian graduates
This is the part missing from most guides written before 2024, and it is the one worth understanding properly.
ECFMG now requires that your medical school be accredited by an agency officially recognised by the World Federation for Medical Education (WFME). Before this took effect, there was genuine uncertainty about whether Indian graduates would qualify.
India's National Medical Commission received WFME recognition status for ten years. That recognition covers all 706 existing NMC-recognised medical colleges, and colleges approved during the recognition period inherit it automatically.
In practice: if your college is NMC-recognised, you clear the WFME requirement. Still confirm your own college's entry in the World Directory of Medical Schools before you register, since the listing is what ECFMG checks.
When should you actually take each Step?
Being eligible early and testing early are different decisions. The sequencing below is what works for most Indian students, and it is built around when the material is freshest rather than when the rules allow.

Step 1 became pass/fail in 2022, which changed the calculus considerably. There is no longer a score to protect, so clearing it earlier and putting your effort into Step 2 CK is the stronger play. Step 2 CK is now the only numerical score programme directors see.
Where the internship fits
Indian MBBS runs four and a half years of academic study followed by a twelve-month compulsory rotating internship, and your final degree is issued after the internship is complete.
That matters for one reason: ECFMG certification requires your final medical diploma. You can sit both Steps before then, but you cannot complete certification, and therefore cannot enter the Match, until the degree exists. Plan the internship as the certification gate rather than an exam gate.
Is the USMLE valid in India?
No, and it was never designed to be. The USMLE is a United States licensing examination and carries no standing with the NMC.
Your right to practise in India comes from your MBBS and your NMC or state medical council registration. Passing the USMLE neither adds to that nor takes anything away, and it is not an alternative to NEET-PG for Indian postgraduate training. If you later complete US residency and want to return to India, recognition of a foreign postgraduate qualification is a separate NMC question with its own rules, so check the current position rather than assuming.
After passing the USMLE, what comes next?
Passing Steps is a milestone, not the finish line. The sequence after that is:
- ECFMG certification. Requires passing Steps 1 and 2 CK, your final medical diploma verified through EPIC, and a satisfactory OET Medicine score as the communication-skills component of a Pathway.

Mistakes that cost Indian applicants time
- Waiting until after internship to start. Nothing in the rules requires it, and you lose a year of runway.
- Treating Step 1 as a score to maximise. It has been pass/fail since 2022. Clear it and move to Step 2 CK.
- Leaving OET to the end. It is part of the certification path, not an afterthought, and a failed sub-test means re-sitting the whole exam.
- Not checking your college's World Directory listing. NMC recognition should cover you, but the listing is what ECFMG actually verifies.
- Assuming USMLE helps you in India. It does not. Decide which system you are aiming at.
